Purchasing an audiometer is a multi-year capital commitment. The purchase price is only the beginning — calibration, maintenance, transducer replacement, and software support costs can easily double the total five-year cost of ownership. This guide helps you ask the right questions before signing a purchase order.

Diagnostic vs. Screening Audiometers

Diagnostic audiometers perform the full range of pure-tone threshold testing — air conduction, bone conduction, and masking — across the full frequency range (typically 125 Hz to 8000 Hz, with extended high-frequency options to 16 kHz or beyond). They are required for any clinic performing diagnostic hearing evaluations.

Screening audiometers are simplified, typically performing pass/refer testing at a limited number of frequencies and output levels. They are appropriate for occupational health programs, school hearing screenings, and primary care settings. If your clinic performs any diagnostic testing, a diagnostic instrument is required.

Key Distinction

A screening audiometer cannot be used for diagnostic threshold testing regardless of ANSI S3.6 calibration status. The classification is determined by the instrument's design and output capabilities, not its calibration.

Transducer Types and Their Trade-offs

The choice between supra-aural earphones (TDH-39, TDH-50) and insert earphones (ER-3A, EAR-3A) has significant clinical and maintenance implications. Insert earphones offer better interaural attenuation, improved patient comfort, and reduced ambient noise susceptibility. They are now the standard of care for diagnostic audiometry.

Bone conduction vibrators (B-71, Radioear B-81) require careful handling and storage. Automatic audiogram interpretation, masking guidance prompts, and speech audiometry test lists are valuable clinical features. Tele-audiology capability — allowing remote supervision of audiometric testing — is increasingly relevant for multi-site practices.

Serviceability and Parts Availability

This is the most frequently overlooked factor in audiometer purchasing decisions. Ask the vendor directly: How long will replacement transducers be manufactured? Are internal components serviceable by independent calibration labs, or does repair require proprietary factory tooling?

Some manufacturers have moved to a sealed-module design that reduces the options for independent service. This forces all repairs through the manufacturer's service network, which typically has longer turnaround times and higher costs than independent providers.

Parts Availability Question

Ask any vendor: 'What is the current parts availability commitment for this model?' A reputable manufacturer will commit to at least 10 years of parts availability. Anything less than 7 years is a significant serviceability risk.

Total Cost of Ownership Over 5 Years

A realistic 5-year TCO model for a diagnostic audiometer should include the purchase price; annual calibration (approximately $150–$350 per instrument); transducer replacement (insert earphone cushions annually; transducer replacement every 5–7 years); software maintenance subscription; and any repair events (budget one repair per instrument per 3–5 years).

For a mid-range diagnostic audiometer purchased at $3,500, the 5-year TCO including all maintenance and one repair event typically runs $5,200 to $6,500. This is an important number to have when comparing instruments at different price points.
