About This Template
This 2-page calibration certificate template meets the documentation requirements of ISO 17025:2017 and is formatted for use with all major audiology equipment categories. It includes all fields required for JCAHO, The Joint Commission, and state health department compliance reviews.
The template is provided in an editable format and includes guidance notes for each field explaining what information is required and how to express measurement uncertainty correctly.
What Is Included
Page 1 covers instrument identification (manufacturer, model, serial number, asset tag), calibration date, next due date, calibration standard references, and environmental conditions at time of calibration. Page 2 contains the measurement data table with tolerance limits, measured values, and pass/fail results for each test parameter.
